Form to get free prescriptions? 2 Months, 2 Weeks ago  
I receive IB and DLA. I did get IS for a while. However, when I moved onto long term IB, my savings were just fractionally too high for me to continue getting IS.

Of course this has meant I now have to pay for my prescriptions.

I'm vaguely aware that there might be a form to fill so that I could perhaps still get my prescriptions free, is that correct? and if so, could someone point me in the right direction?

Hi,

Yes if you have a low income, you maybe entitled to help with prescription and other NHS costs.

You can pick up an HC1 claim form from a Jobcentre Plus Office or NHS hospital. Your doctor, dentist or optician may have one too. You can also request a form by telephoning 0845 850 1166

or request one via this link:

Alternatively, as you receive HRM DLA there is a chance you may qualify on grounds of you need assistance to leave your home. You should ask your GP about this as there is a form he/she can fill in to enable you to claim free prescriptions on these grounds.

Thanks Pete - I'll look into that. Well remembered about my HRM!

I do in fact usually have to get a taxi to go and pick up the prescription so it's a very expensive business!

If you satisfy the following then you can get free prescriptions.

"you have a continuing physical disability which means you cannot go out without the help of another person.

ask your doctor for Form FP92A.
