My DLA runs out in September and in the process of filling it out, almost there thank goodness (had it since April & its taking me forever) Unfortunately the form isn't geared up for my illness, so trying to making the form work for me has been an absolute nightmare :(

What i would like to know that since the beginning of last year i have had many appointments and treatments from different sources but as present without any relief from pain. Do you think it would be useful to send all copies of my appointments with my DLA form including copies of the prescriptions i had for injections into my head at the royal free? I have also requested my medical records from my neurologist but as yet they have not arrived but will send them on afterwards as i need to get this in the post within the next few days! I also received a letter from my consultant today confirming that i have been put forward for botox injections as i haven't responded to any other treatment?

All this paperwork is relevant to my condition but do you think its too much to send?

If you believe that these documents testify to the severity of your problems, then you should consider including them with your DLA forms.

The issue with large quantities of evidence is not usually the volume but the organisation, ensure that each piece is clearly labelled and create an index of documents to make it easier to find something.

Thank you Gordon... Its going to cost me a fortune photo coping all this evidence as well as the actual DLA form?! :ohmy:

I sent all hospital appointment letters, and every other piece of paper relating to all/any of my illnesses.

Remember to write your name and National Insurance number on them all. Also write the page numbers on them e.g. if you are sending, say, 20 sheets, number them 1/20, 2/20, 3/20 and so on.

Also send it by Recorded Delivery, so that you have proof that it was sent, and received by the DWP.
