face to face for mental health and serial 7 test

I at a ftf asmnt with my 26yr old son ( depression/social anxiety & poss other issues to which he refuses to acknowledge). The HP said she used to be mental health nurse with 10years experience (working nights). At the end of an hour and 20mins she said to my son HP thought he was engaging with her face to face and verbally fine (this was because I got him there, I was with him and he knew he 'had to' as a one off). I was taken aback by this but before I could say anything HP asked him 3 questions, i believe, out of what I have since discovered is the serial sevens: 1) to spell world (he did) and to spell backwards (he couldn't) - 2) to count from 100 in 7's backwards (he got one wrong after much struggling) 3) asked to repeat 3 random objects and asked again at end (he got one wrong to which HP said don't worry am sure it was my southern accent). My son got upset when he asked why she was trying to unsult his intelligence, HP stated she wasn't it was to check his concentration. This really upset him as he is adhd and his concentration is awful. Do you know of anyone else who has had this and is this fair/reasonable?
