OMG copy of capita medical report pip

i have received a copy of the capita medical report I requested last Wednesday OMG I can not believe what I have just read there are things on there that weren't discussed at the home visit and the information that was is not included it is unbelievable she has got all the dates wrong with when I was diagnosed with arthritis and what dates I had xrays I also gave her new information about my condition [that was not included in the original form]ie blood tests for rheumatoid arthritis wich she said she had included but no mention in her report and also goes on to say [she does not have any ongoing treatment at this time.Also she agrees to a point regarding me suffering with depression and anxiety, I also told her I had been in hospital in the past because I had attempted suicide and I have suicidal thoughts often but on her report she states she has not had any hospital admissions relating to her mental state and there is no treatment plan when I told her I am awaiting an appointment with a mental health team she said I am able to converse with family and friends face to face and on the telephone daily this was never discussed she said I was dressed appropriately for the climate I was in jogging bottoms ,nightdress and cardigan and it was 1pm. she said at the assessment I appeared calm and relaxed I told her when she arrived I was very anxious and had been physicaly sick that morning before she arrived no mention of that . She said she observed me walking around my living room she didnt she said I could walk 200 yards I didn't I said 20 yards she asked me about planning a journey I told her I couldn't that's why I didn't go to the medical in Cardiff 90minuites awy because I could go on my own my daughter told her im unable to go out alone because of severe anxiety and I get in a state and because my daughter knows what im like she is best placed to be able to calm me down and reassure me but in her report she states I reported to her im able to go out shopping most weeks there is so much more but I would be here all day writing so I have rang to request the decision maker to ring me to discuss this report because im angry upset fair enough I understand that these forms have got to be filled in and ok a ftf assessment is needed but to not put the true picture of the persons disabilaties and how it affects their daily life is unbelievable to say the least so my advice to any one considering a mandatory reconsideration is ask for the medical report this has made me so angry but has given me the extra push needed to prove these people wrong regarding my state of health and how it affects me daily

so should I go through the report pick out the factual points then give my reply to each of them ie 1 to 10 and put my points to each one just incase the decision maker does not ring me as I have said in one of my other posts I have got an appointment in the morning with the mental health team and im going to ask them for a report or letter to confirm my mental state and then I can include this with the above information and also sorry when I had to have a home visit because I could not travel alone to the medical 90mins away should they take note of that thank you

Gordon's given you good sound advice, especially about concentrating on the facts.

Remember that the DM doesn't know you & won't have seen you so try to paint a mental picture for him/her of your day-today life.

Also rather than slating the assessor or trying to make them look incompetent you could say something along the lines of they only saw you for a snapshot of time which is why you feel that the assessment wasn't a true reflection of how you are able to function on a day-to day basis.
