ESA Review claim form & notes about how to claim

Good day to you all.

Latest and need help please.

I had two phone calls from DWP on Wednesday and one on Thursday, all leaving messages.
The first two message said 'You were not able to be contacted today', there was no tel no name or extension given, just 'you were not able to be contacted' and the third message said 'You were not able to be contacted so we will write to you'. Again no name, tel no ,dept or anything.
Pretty useless really. Could not contact them cos no idea who or what for.

Saturday morning thru the post arrived as 'Employment & Support Allowance form which says Review Claim form & notes about how to Claim.

The letter along with it says it is a ESA2 form and you have to answer all the questions by ticking the 'yes' or 'no' boxes.

I have one week to do so.

It is a threatening letter stipulating that one must deal with it or one will be have benefits suspended and says make sure you fill it all out properly otherwise it will be returned to you to refill it out if any questions are not answered, yet it says do not answer all the questions at the same time !!!! . Oh and one must not cross out any sections or pages.

I shall start to complete the form and ask for help as I go along.

One section is daft as it says please name the doctor who signs your medical statements(also called sick notes, medical certs, doctors statement)
But if you are in the SG you do not need such statements and moreover our practise is a practise who do designate a specified practitioner.
As law states at the moment within the NHS, designated practitioners are only provided to the elderly - who will not be eligable for ESA !!!

It asks for all your income etc and a load more intrusive questions.
When one is in ESA SG CB savings are not their concern, so why do they demand the information, along with any disregards if they are below the £85 per week as a pension.

1 section asks for your name & address, the following page it asks where to send it to if you are homeless and what address to send it to, but I have already provided them with my permanent static home address on the previous page & I am not homeless or ever have been. !!!

I presume lots have already had or are awating such a wonderful form to complete.

I am in the SG CB ESA running from Oct 2013 to Oct 2016 - 3years.

The form and letter are so very wonderful if I were not reliant on this peice of junk I would respond in a fitting manner !!!!!! :woohoo:

Hi pcm,

An ESA 2 Form is similar to an ESA 3 Form : Sample ESA 3 Form

Have a read through : This Topic.

As with an ESA 3 form it is used to review your personal and financial circumstances to make sure that they have not changed and you are receiving the correct amount of "benefit".

An ESA 2/ESA 3 is not part of The ESA WCA, and is not used to review or determine which ESA "Group" that you are in. (WRAG or SG)

The phone calls may have been to carry out a "review" over the phone, but they have now sent the ESA 2 as they could not contact you.

Re : the "sick notes" question, simple write NA, I am in The Support Group. (SG)

You should fill in and return the form by the deadline date as instructed.

Sending Documents to the DWP.

As the form requests information about your personal and financial circumstances, there is not really much that we can help with.

Hiya,

Many thanks.

Yep I know I must get on with this short nicely worded simple form.

The accompanying letter is extremely threatening and would really upset many with the tone of the letter.

I am having a meeting with my MP at home the week after the budget, organised a while ago, to see where we go with the impending cuts etc.
I shall keep a copy of the letter and form back for him to peruse.

I suppose this is instead of one of these compliance meetings that some seem to be having.

Good luck to all out there especially when you get one of these fabulous forms that are soo easy to comprehend.

We have just completed filling out the form and found many poorly worded sections.
We have mentioned in section 20 how bad the form is and why we found it confusing.
This will probably be a reason for a sanction !!! :blush:

I do hope any one else who receives one of these forms does not find it as bad as we did or indeed finds someone who can complete it for them.

Husband has photographed - photo copied in the form for future reference if needed.

Yes , me too felt the form a bit threatening, albeit a smaller form demanding pension information and dire threats if not returned in 7 days. As Gordon mentioned this demand from ESA cb clients for financial information seems to be escalating.

Good morning to you all.

Today 2/7/15 I received a follow up letter from the ESA2 Review form.
They require- in the letter- Latest Pension statements from Herts CC Pension and Pension Statements from 2011 - 2014, but they require ORIGINAL Documents.????
Has to be done by 14th July, no need for a stamp etc etc along with the dire threat of suspension if you do not procede.

NowI do not mind sending copies but really do not like the idea of sending originals giving their state of loss within the Jobcentre plus etc .

The last ESA2 I sent in recently, I did still pay for recorded delivery, but it gets rather expensive continually sending things to them recorded delivery, - which we all have to do as they are so well equipped at stating they never received anything if one does not record delivery anything to them.

So should I send the required Originals, pay yet again for recorded delivery in their prepaid envelope, or just send it in their prepaid envelope and hope for the best.

I have written in the 'reply' page that I am not happy in sending them originals and that I require them returned immediatly upon being read.
As the law states one must keep these records and not send originals .??!!
Do they not recompense if you yell at them?? Oh, I mean calmly write a please and thankyou letter to them !!!!

Looking forward to any replies pertaining to this query.

Why are they sending these ESA2 forms to those in ESA SG CB??

Do I or others need be concerned in regards to this matter ?
