Hi Gordon

We are making the initial PIP claim tomorrow via phone. I have a query regarding PIP1 form - questions they will be asking my wife (she is my daughter's DWP appointee).

PIP1 asks: 'if you are filling the form in for someone else, tell us about them not you.' So, this will be our daughter's name being entered here. However, a few question later it asks for her phone numbers. Should we put down my wife's phone numbers as my daughter doesn't use a phone due to her limitations? Incidentally, further down the form it does ask for the appointee's phone numbers.

Also, they will ask 'do you need communications in large print, braile or audio CD; my daughter obviously would normally as she's blind. However, my wife doesn't need any of these. But like I said this section on the form is specific to our daughter? Any advice. What I'm worried about is my wife stating she doesn't require a different communication format for our daughter - and then they interpret this as meaning - no problems with her vision then. Surely, I shouldn't be confused/paranoid already. Many thanks.

Where the questions relate to your daughter you can write not applicable, appointee does this for her.
Then your wife can fill in her details wherever it asks for them. In this way you are covering the fact that your daughter uses her mum as appointee all the way through. Hammer it in and it should have the desired effect.

Hi Foss, thank you for your reply. One slight problem - we won't be filling in the PIP1 form - it will be oral answers given over the phone. Should this be a problem?

Also, I don't know if you are aware, the PIP1 (DWP official latest online sample copy) has been amended this year; it now asks about any DLA you may be getting (page 18) in addition to ESA. The form even suggests that ESA is part of the DLA process?

I'm tempted to tick the box (oral answer) that says: 'I want you to use all medical evidence from my DLA claim.' However, I have one small concern. One of the supporting documents I submitted with a past DLA claim does have a noticeable contradiction regarding the extent of one particular difficulty. It was an error from someone who had met her once. The same document does state the correct fact (other medics). Any advice?

Am I right in thinking they will look at absolutely everything anyway regardless - as the declaration seems to give them permission to do anyway. Therefore, I'm probably worrying about nothing as it's out of our control anyway. Any thoughts?

Hi Buster,

matchseller makes some good points.

I have worked as and with professionals who fill out PIP and ESA forms. I do not and cannot offer this service now. However even for a person who may be filling out up to a dozen of these forms a week it was still up to a 2 hour appointment for the claimant and carer.

So take your time.

I suggest writing your answers in rough before committing them to the actual form itself. Also talk over your daughters condition with her.

My experience is that claimants will often put on a brave face and down play their discomforts and incapacities, even to relatives and carers. Whilst personally many see this as commendable, it is not a good attitude to take when dealing with the DWP. They have a tendency to downplay conditions as a matter of routine.

Even though you may decide to allow the DWP to use your previous DLA information, it does not mean that they will. Old evidence is regularly destroyed as part of their Data Retention policy and even if it is available, the assessor is not required to review it. Our advice is that if you want any evidence to be looked at then you need to attached it to the PIP2 form.

As to the PIP1, I think you are overthinking this, it's just a ten minute call to start the claim. :)
