ESA 50 form and asking for a tape recording.

Hi all. I am already on ESA and have been sent a ESA50 04/15 form which has to be sent back within the next few weeks. Please could someone direct me where on this site I can find the best way to fill it in. Any advice will be helpful. Thanks in advance.

Everyone in receipt of ESA will be reassessed periodically. Our ESA guide talks you through the process. You should approach the form in the same way that you did your first application as it is unlikely the assessor will have access to your previous paperwork. Complete the form in detail and supply (copies of) all supporting evidence.

How do you all feel about asking for a recording on the ESA 50 form just in case a person has to attend an assessment for ESA. Or should it be asked for when an appointment is made? Thank you.

We recommend asking for the assessment to be recorded when you send in the ESA50 form. Check that this has been actioned if/when you get an assessment appointment.
