"you do not have to discuss with or show the information to the patient before you send it."

So no instruction not to discuss the matter with the claimant.

Not for the IAS f2f assessment, but the form sent to GPs etc.

Thanks, Gordon, I knew that info was in the guide but wondered if there was an actual template/blank copy I could print out. I'll just copy and paste it into a Word doc.
I also found the GP notes in the link BIS gave helpful - will print that out for GP.
