I've searched gov and B&W sites, and can't find a template or example of the PIP Factual Report - is there one available for the public/claimants to view?

Not for the IAS f2f assessment, but the form sent to GPs etc.
Thanks, Gordon, I knew that info was in the guide but wondered if there was an actual template/blank copy I could print out. I'll just copy and paste it into a Word doc.
I also found the GP notes in the link BIS gave helpful - will print that out for GP.
