yes. Would that mean I need to look at drafting a Universal Credit form or an ESA 50 form, or both just in case as I would like to prepare myself mentally really well in advance and obtain support letters from GP's and Consultants before November 2019.

It would be UC for the ESA(IR) element but it is an online form, I'm not aware of any downloadable version.

I realise you want to be prepared but the Managed migration of ESA claimants is not even a defined process at this time and whilst it might start in a year, it could be many years before your daughter is actually invited to claim UC and who knows what changes to the process will occur in this time.
