We can provide some support but I am afraid you are going to have to do much of the work yourself.
Have you tried to get face to face support? Do an internet search for "welfare advice" with your postcode town or county, you can also try contacting any charities that specialise in your conditions, as a final choice, the DWP do provide a form completion service.
Our PIP Claim guide explains the criteria you will need to meet and how to complete the form. see
