Dear Gordon, Mrs Hurtyback,
Just gone through my telephone PIP claim and now waiting for the paperwork to arrive. I have been looking through members guides on filling in the forms, But can see Dialysis mentioned anywhere ??? My age is 68, I am on CAPD home dialysis 4 times per day, every day. This takes me 1.5 hours each time, making 6 hours per day hooked up. Plus all cleaning, wound dressing, deliveries, waste collections, ect, I find it impossible to keep to appointment times because my treatment times vary day by day, and my health is up and down. Please tell me if you know ? how do I ask for a home visit.

The Dialysis would come under Managing Treatments on the form.

As to a Home Assessment you will need your GP to write to the Assessment Provider stating that you cannot attend at their offices and why. It's very important that there is no suggestion of circumstances in which you might be able to, so phrases such as "may suffer..." or "could..." will invalidate the letter.
