There's no point writing you want to be paint the ESA Assessment rate on the SSCS1 as it now goes to the Tribunal Service, it used to go to the DWP first.

Once your appeal is accepted and you have received notice of this then contact the DWP office that dealt with your ESA claim and ask to be moved from JSA to ESA at the Assessment rate, they will need to have received notification of the appeal first, so you might want to wait a couple of days. before phoning them.

You will need a Fit Note, ideally from the day after you ESA ended but as you were receiving JSA this is not critical. The Note needs to be sent to the same DWP not the Tribunal Service.

You should receive confirmation that your ESA will be reinstated.

If you haven't completed the form properly then it might but they will write to you asking you to correct any error, otherwise I can't remember the last time that I saw an appeal rejected.
