My wife was on DLA but recently decided to claim PIP as she went into hospital and Consultant said she had COPD. Rally bad breathless etc.
Anyway we had an assessor come to our home from Capita. The interview had to be cut short the Capita aSsessor was actually helpful. She said that we had described my wifes symptoms and how affected her BUT nowhere did on our form di we say what her "Medical Condition" was.

The discharge paper refered to cachexia, oedema, pleural effusion but she stated these were symptoms not a Medical condition. I got a medical report from Doctor back to 2011 but still did not state medical condition. I wrote to my Doctor asking for a letter to spell it out that I needed a letter stating what her Medical condiotion was I.e. COPD. But she will not do a letter !!

Anyway should I write to hospital to get report and/or letter. It may be that hospital may still not show that she has COPD ?

The Hospital should have written to her GP with the diagnosis, the letter should be on her medical records which she can request access to. See

Finally trawled through the 120 of pages of wifes medical report from Doctor and found something Dated 15th November Date admitted to hospital. Over the page it states "The following diagnosis was made "Chronic obstructive pulmonary disease (Suspected Diagnosis).
Hope that is good enough ?

Was was in hospital for about 5 weeks discharged 17th December. Doctor verbally in hospital refered to COPD but not mentioned in Discharge letter. Discharge letter refferdd to Cachexia (Muslce Wastage) and Pleurel Effusion. Which I gather are symptoms.

Anyway willpost off as extra evidence.
