Hi,I have received a letter asking me if I want my ESA claim reinstated.
Would this mean I go straight back into the Support group or would I need to fill in another form and have an assessment ?

Is this a follow on from the appeal that you won, if it is then they should place you in the group awarded by the DWP.

If is not, then it will depend on why your ESA was suspended in the first place which you do not mention.
