I'm just about to appeal against a (DLA to) PIP decision (having had the reconsderation). Should I ask for a copy of the medical assessment report?

You don't need to, when your appeal request is accepted the DWP will be notified and will prepare their submission, this will include all of the documents included in the making of the Decision, one of which will be the assessment report.
