DLA-PIP writing for someone else

DLA-PIP writing for someone else was created by wilk
My husband could not possibly fill in this form himself due to his problems after a brain injury. If I am filling it in for him, (and he will sign it after I've read my answers to him) in what context should I write it?
I have previously for esa used "I" and "my" but am just wondering if this is correct?

If you are using his descriptions of his problems then use "I", if it is your words then use "he".
