My PIP claim ends Jan 19, been sent new claim form(1003) is this the correct form even although i have b awarded before.First award was 2 years.Thank you.S.

Yes, this is the correct form.

All awards of less than two years and some of this length are not reassessed but are renewed, this is done using another PIP2.
