Hi, just re reading our PA4 report and we see a lot of 'codes' in the 'justification for descriptor choice' section
so what do..
CQ
FH
MSK
IOs
SOH
Mean?
Pleased to report that we are progressing well due to the help were getting on here!! but would really help if these codes could be explained so we can make the best case possible in the MR

FE – Further Evidence
FH – Functional History
FME – Further Medical Evidence
FRR4 – Telephone contact form
HOC – History of Conditions
MSE – either Mental State Examination or MusculoSkeletal Examination or Medical Services Examination depending on context
MSK – MusculoSkeletal (probably)
MSO - Medical Services Observations
IO – Informal Observations
OPD – Overwhelming Psychological Distress
POI – Proof of Identity
SOH – Social & Occupational History

MSK is right by the way! ive seen that word in the medical report so explained
this will now help us to make our MR as robust as it can be!!!!
