PIP Mandatory Reconsideration request was created by Ellieg

Hi, I requested a Mandatory Reconsideration of my PIP assessment last month within the time specified. I wrote a letter with the help of the guides for PIP appeals detailing the reasons for my request, Can anyone tell me if I should have included some official form with my letter as I wasn’t told that I needed to when I rang them. I would appreciate any advice before I ring them again. Many thanks

There is no requirement to use a form o request an MR, a letter is fine as long as it is clear that an MR is being requested.
