What evidance should i take to my ESA medical

I Have been looking at this page to get ideas of things to take to my medical www.dwp.gov.uk/publications/specialist-g...ces-of-evidence-a-z/
I am a little confused by it.
So far my plan is to get a letter from my GP, A letter from My local mind and take letters i have recieved from my specialists and discharge notes from when i have had tests or been in hospital..

should i take anything else?
I have been on another forum and they suggested taking,
A letter from my mum who looks after me,
A letter from a close friend,
And guide lines of the illnesses i have been diagnosed with from the NICE.org.uk website.... Iv been thinking it might be better to take DWP guidelines tho not sure.

Your help would be very much appreciated

Hi M89,

It would always be advisable to send any medical evidence such as you mention above, with your ESA50, it is questionable whether the ATOS HCP would have time to take into account any evidence that you hand to them at the time of the face to face.

A lot of members have also sent copy of their ESA50's and any additional evidence to the DWP DM, to ensure the DM has it in front of them, as it is the DM who actually makes the decison on an award of ESA, the ATOS HCP only makes recommendations to the DM.

Any evidence that would show that on the balance of probabilities you suffer from the limitations that would qualify you for an award of ESA under the relevant descriptors and criteria should help.

what probabilities do you mean by this 'Any evidence that would show that on the balance of probabilities you suffer from the limitations that would qualify you for an award of ESA under the relevant descriptors and criteria should help'?

Okay, Just looked up dwp dm. so after i have my medical will the information be sent to the DWP DM? or will i need to send it to the DWP DM seperatly?
I was going to ask them to take photocopies of the things i take so when my medical report goes to the DM they see it then?

Hi M89,

There is no need to take evidence to a face to face assessment, that you have already sent with your ESA50, the examining ATOS HCP, should have your ESA50 at the medical assessment.

ESA Medical – What to Expect

A DWP DM, is a Department of Works and Pensions Decision Maker.

This is the person who actually makes the decision on whether a claimant is entitled to an award of ESA, and if so which award, WRAG or Support Group.

The DWP DM will make the decision taking into account all evidence in front of them, which may include, evidence from the claimant, the claimants ESA50, any medical reports from a G.P. such as an ESA113 questionnaire, and any recommendations made by the ATOS HCP either as the result of a face to face medical. or just as a paper exercise without a face to face.

The DWP DM makes the decision on "The Balance of Probabilities", rather than beyond resonable doubt.

In other words is it likely that the claimant fulfills the criteria of the relevant descriptors for an award of ESA.

All this is laid out in our ESA Guides accessible from the link that I have provided.

Unless that you have actually received an appointment for a face to face, it is not definate that you will have to attend, many of our members have received an award of ESA, without the need for a medical, as they provided enough evidence with their ESA50, for a "paper scrutiny assessment"

I have read most of the information on the website. there is soo much of it to take in tho it is all soo confusing.
I sent my ESA50 in back in september or something like that. SO i have newer up to date information.

If i ask my dr if she has done an ESA113 questionair would she tell me?

I have recieved an appointent for a medical for the 10th of january so next tuesday.
