appeal form

can anyone please let me know about the appeal form. I have completed one that needs to go off today. however I down loaded the form from the site and the last page states to sign over the page but I can not find the other side on the form on the site. do I just date and sign at the bottom of page 8. There is a letter going with the form

Hi J,

It is just the format of the downloadable Pdf version.

Don't forget in normal format it would be in the form of a leaflet.

As long as you have signed and dated it at 6. on page 27 of the Pdf, that is fine.

If you are using extra pages, draw reference to this in the reasons box, and put your name, NI number, and DOB, on any extra pages, and sign and date each one also.
