I have the form now with a date of 27 October and told my Doctors I have a deadline so hopefully my notes should be sent this week. I noticed on the form you can ask for an email version of the form. This would be much simpler for me to fill in is it worth asking for an email copy of the form?

We've not heard of an email'd copy being available, the latest information from the DWP is that there is still not an interactive version of the form, it may just be a copy that you have to print and then fill in.

So I have had an extension to the deadline as the earliest appointment I could get with Age UK was tomorrow as they are making a home visit to help me with the form. I have spotted a mistake in my medical notes saying my dilated cardiomyopathy is resolved but checking this on the Cardiomyopathy Association specialist nurse they have told me this is uncureable and a lifelong condition. Is it worth writing to my consultant and getting this corrected?

The likelihood of your conditions changing is a major factor in determining the award length, so it is definitely worth your asking your Consultant whether they will change the letter.

Is it normal to receive a deadline reminder a week after I phone up for an extension to the deadline?
