DLA to PIP letter was created by Teri606
Help please, I'm panicking already. I need to ring the PIP people and say that I need to apply for PIP. I have had HR DLA indefinitely since a bad accident and surgery to my back in 1993. My last form which I filled in was in 2005 and has continued since then. I have all the information prepared which is stated on my letter but wonder whether they will go into any details about my present problems (which are the same as in 2005) or if it is just a straightforward telephone call with nobody putting me on the spot about the effects my problems have on my life. It depresses me terribly when I have to relive the details of my accident and subsequent surgery so I'm trying to put that off until I get an actual form to fill in.
I will start to read up all about PI again.

Thanks for any advice, Teri

Have a look at the PIP Claim guide, it explains the questions you will be asked on the initial call.

I have listed my conditions and meds in question 2a & 2b and intend expanding on the conditions and problems with my meds/treatments in A4 documents as part of Q15- Additional Information.
Is it OK to do this? Some of my meds are also mentioned in the explanation of my conditions.
Is this overlap OK or is it expected to keep conditions and meds entirely separate.
I have needed to put an explanation of one of my conditions as it is an extremely rare syndrome (only 100 people worlwide) which caused 4 other conditions.
I need to do my additional info on my laptop as I cannot sit at the table to write the form and my handwriting is awful anyway.

Just make sure that each extra page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.

Attach the pages to the back of the PIP2 using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.

Thanks for the info Gordon. I am doing this as I go. The beauty of Word is that I can keep going back and adding things as I remember them.

I'm having difficulty finding the words for question 13. I never leave the house unless it is in the car. I can drive myself with confidence about half a mile away to friends. Otherwise my husband is with me and either of us will drive. However, my husband has to tell me where to turn left/right etc. if we're more than a couple of miles from home. I have no sense of direction at all and can never remember how to get from A to B unless it is near my home. If I were to go out on my own, and got lost I would not be able to ask for directions as I am really quite deaf. I have not been on a bus in 40+ years and would not have a clue where/how to get a bus. Is any of this relevant please?

Regards, Teri
