I'm afraid the letter reads, to me at least, as if it is you saying you cannot travel to their offices, rather than the GP saying it. The phrase "She has expressed..." or variations of same is used by GPs to say "I don't have a clue but my patient thinks they have a problem".

You really need a stronger letter where the GP specifically states that in their opinion you could not travel.

That’s exactly my point Gordon my GP just will not help me I don’t understand why his answer was he doesn’t have time to deal with benefits I spoke to mental health team myself last night and they said he has to supply medical evidence it’s his job and it is all clearly noted on my notes my mental health status

Unfortunately GPs have no obligation to provide patients with information for their benefit claims, just the DWP if requested and most of them don't even do that.

Well I got a call today from my surgery saying dr had done a letter for me to collect to say I was shocked is an understatement

Dear sir/madam

Miss ?????????? Is registered with ??????
She has long term history of having post traumatic stress disorder and anxiety with depression and agoraphobia. She used to be under the mental health team and having medication she is not under mental health team at this present time bid she has expressed that her mental health has not changed and she is finding it hard to cope she also suffers from back problem suffering terrible pain which she is under ??????? Hospital and is awaiting surgery on her back she is currently on morphine for her pain
This information is produced on patient request to support her for pip application

That letter is worse than any others so far he now made me sound like I’m not even on medication for my mental health
1) it isn’t my back it’s my neck DDD stenosis of spine and cervical ruptured and herniated discs
2) I am only not currently under the mental health team as I have been stable for two years with no self harm or suicidal thoughts or attempts except Hospital admission in August for a panic attack
3) he has clearly put in there that the letter is at my request

I guess I am just gonna lose any appeal I attempt due to his lack of help

Can you label your posts as to whether they are about your upcoming ESA assessment or they are about your failed PIP claim, as I have a feeling that we are talking at cross purposes at the moment.

Re your PIP, have you requested an MR yet? Success rates at MR are low but are over 60% at appeal.

Oh dear, I really feel for you.
I was awarded 0 points for care component of PIP, despite previous high rate DLA. I went to tribunal in court and won my case in January, 2017. Am now awaiting ESA review with trepidation.
Like you, I have both physical and mental health needs and sometimes all my into gets very muddled up.
One piece of advice that I used in my written application is the repeated response 'i am unable to undertake this activity reliably, repeated, safely and in a timely manner due to (give name of illness, then specific to this question how your body would respond/react).
If I have to go to a medical, I will repeat these words in answer to each question, before I give a specific answer.
I will repeat these words until the Assessor is as sick of me as I am sick of the process.
Take a very deep breath, re-read all the advice give and if possible have someone with you to keep as calm as possible throughout each step of the process.
