It's their claim and only they profit or lose by the result, so if they want to do this then they should, just make sure that they understand what the advisor has written before they add to it or correct it.

Thank you Gordon. We were certainly not intending to change anything that adviser wrote, but maybe add something on a separate sheet. Shall certainly do as you suggest and make sure that we read and understand everything that adviser wrote. Shall tell my friend what you have advised. Thanks again for pointing this out before we make any mistakes!

For sure! CAB woman made a copy but I suggested to friend to scan completed form and keep her own copy too.

Would be grateful for some suggestions. PIP form filled in by C.A.B. advisor. Friend was waiting on 'GP notes' form back from doctor. What to include?. Friend gave GP 'GP Notes'. GP called my friend and said that the notes are too long. GP is happy to include a letter but my friend needs to write out a shorter version out of 'GP notes' and she will sign it. Any helpful suggestions on how my friend should word this "shorter version". Thanks

The only way to reduce the GP Notes is to remove those activities that they don't expect to score for, if try and reduce whatever is on each page the you risk being seen as putting words in the mouth of the GP, which will make the report ineffective.

As an alternative to using the Notes, the GP could base their letter on them, however, given the GP Notes is primarily a tick box exercise you have to wonder how much they are actually intending to write.
