hi again and thank you, i see what you mean. having the new diagnoses of ischemic heart disease hasnt changed my day to day living and there hasn't been any change in how much help I get ie dressing bathing preparing food ect. the changes it has made, however, is when i get extremely stressed out and anxious. i suffer from anxiety anyway but extreme anxiety can give me an attack of angina, for example, when I had to attend the cardio department, its extremely difficult for me to get out with the agoraphobia but, having to go to the hospital as well as going out, brought on a severe anxiety and panic attack which brought on an angina attack. that was caused by having to go out, to a public place and having to see a consultant that i didn't know. another thing that can bring it on is, example climbing the stairs which i very rarely do anymore and never on my own, what was mentioned in my last review, the assessor mentioned that i had an electric bed and was trying to get a stair lift installed, i had an occuptionl assessment not long before my face to face who was tring to get this sorted for me because i struggle with stairs and i sleep downstairs the majority of the time because the pain is to severe to climb the stairs. unfortunately, my stairs are not wide enough to have the stairlift, my bathroom and toilet are downstairs to so, i dont have to go up stairs to use the bathroom. climbing thestairs can bring an angna attack on, but these where things that werent even mentioned in my last review and didnt know i had angina then, i always put it down to my copd and getting very breathless. i dont use the upstairs of my house very often but on occasions when i have been able to go to bed, my daughter helps me up and then back down in the morning, i dont go upstairs very often, it all depends on my level of pain, so its not a major issue so wasnt disccused at my medical or on my form really. i will take another look on my form but im pretty sure climbing the stairs wasnt put down on my form with spending the majority f the time downstairs. I do know it was mentioned on my esa form because it was a question they asked.

PIP considers your problems with the activities on the majority of days, you should also have a look at the reliability criteria, both are explained in the PIP Claim guide.

Hi again Gordon.
I received my review form yesterday and you said, it is a form asking if things are easier, the same or harder. I have just ticked the boxes for now and downloaded the dear gp for. I did photo copy my PA4 v3 report form which my daughter took down to the surgery in may as i am also going through review of my ESA. I m still waiting to hear from them, my form was sent back next day delivery on the 15th may. My form apparently is still with the medical team.
I have been in touch with my doctors this morning and ask my gps secratary if, it possable for my gp to fill in the dear dr form using my consultation report that they have on file for me. My gp doesn't know me very well at all, she does, however, know how my COPD affects me and also the heart disease. as for my other conditions she doesn't know how they affect me at all really, she can only read reports that my gp who has retired has written. Im hoping that my PA4 V3 will help her to fill in the dear gp letter as i cant get into the surgery. would her filling that in be helpful for me? I do really need to see her because my barevement is affecting me terribly, I cant ask for a home visit because its not a medical condition.

If your GP completes the form then yes, I think it should help you with your reassessment but don't just send it, make sure that you are happy with what they write.

as always I very much appreciate your advise and reply. If im not happy with what my gp say, can i ask her to correct the parts i think, could cause more damage than good, as in my earlier messages, she really doesn't know me very well at all, she knows nothing about how my pysical health conditions affect me but is, aware of the conditions of my severe COPD and heart condition as she has been to see me with both of these and how long i have had them for.and the severity. she did say my COPD alone was bad enough and if i had to i could go to tribunal? this is something i want to avoid because i really don't have the strengh to take them on.When i last spoke to her about my review coming up and also mentioned i was worried because she didn't know how my condition affect me, she was a little rude and said, then you will have to tell me won't you. She didnt want to write me supoting evidence for my ese when i ask she said they will write to her with any questions and that this is now something that they do in most cases, now?that s why i sent my medical PA4 v3 copy so it wa on there system so it gave her an indecation of how my conditions affected me when she reads the medical assors report and also where i got the ticks on the quetions why my medical assesors thought i met the criteria. I didnt know my gp would have to write about me i though she could just tick the boxes for which criteria i fitted into, ie neededs assisted with dressing washing preparing food? ect like the medical report assors had said..\maybe its me. i ony buried my son on the 15th so my heas allove the place t the moment, the last thing i needed with grieving very badly was,having my esa review and now this one before i have even had any aknowledgment from esa. Not that they care waht your going through at that moment in time, they must think were made of steel? sorry if this is all muddle up, i am trying to write this and have taken a sleeper, im falling aslleep but will rea your reply tomo.
