MR for PIP Help

Can someone explain why my PIP decision report consisted of only two paragraphs with no reference to descriptors or ( justification of descriptor boxes etc )

Totally different to consultion report form PA4 as used by Atos .

I have not been told verbally or in writing the profession of my Assesor and who he works for.
This is my 2nd pip assessment by the same person and it looks like something from Jobcentre Plus .
No Current medication and treatment , no social and occupational history, no functional history,
no formal observation.

Any advice would be greatly appreciated.

Yes it’s a decision letter I gather as it’s awarded me standard rate of £57-30
but it’s so awkward to unpick just a few paragraphs to appeal as the information given doesn’t reflect what was said in relation to some activities.

Sorry yes I did have a face to fave with my sister and Support worker present.

The form doesn’t look anything like a PA4 that others receive.

I had a home face to face and this is the decision letter that awards me Standard rate.

I can’t understand why it’s so short and doesn’t give each activity with a Justification like a PA4 that others get ?
