capability for work questionnaire

Hi, I have received a capability for work questionnaire this morning, and am a bit confused. I already receive esa support group, and dla higher rate mobility and lower rate care. So why have they sent me this new form now ? It's almost as though I've got to apply again, but it doesn't mention which benefit it's for.

I am assuming that it is The ESA50 Questionnaire !

You can compare it with The PDF ESA50 obtainable from the link further down this post to confirm this.

ESA in either group (WRAG or ESA) is not awarded permanently.

With each award come a recommendation of when your ESA award should be reassessed.

The maximum time before reassessment is "usually" 3 years, but you can be reassessed at any time after 3 month from the date of your award..

It looks like your time has come to be reassessed by : The Centre for Health and Disability Assessments (CHDA) (Maximus) who took over the ESA WCA Contract from ATOS on 02/03/15.

You should be aware that there were changes to the qualifying criteria in 2013, so you should treat this as if it was your first assessment and provide as much current evidence that you are able to.
