Esa completion form was created by Moosemouse
Hi I sent my esa 50 back with evidence letters from hospitals/doctors. 4 weeks ago I have just received a letter from my gp asking me to attend on Friday to do a esa completion form which she has been sent

Any idea what this could be or what form the gp has been sent ?

It will be an ESA113 with questions about your conditions and limitations.

Thanks it was a esa 113 form ,I filled it in with the doctor, will I still have a face to face assessment ?

Or will I be passed on based on the forms I have sent
