Hi L944,
I am not sure which documents that you have requested.
Having looked back on your posts, I see that you were placed into The SG for 18 months on reassessment, without a face to face.
In that case, there will definitely be an ESA85A Medical Report, there will also be an LT54.
There may be an ESA113 if one was sent to your G.P. So the only certainties are The ESA85A and The LT54.
I don't know whether you made a formal request using a SAR, if you did, The DWP only have 40 days to comply.
If this is the case, you could remind them with the threat of a complaint to the ICO.
If you didn't issue a SAR, you could do so now.
You could directly name The ESA85A and The LT54, and then add on "ALL" documentation that was used in coming to the decision.
If you particularly want to know which SG Descriptors were applied in placing you into The SG, then ask that particular question also.
The same goes if you want written confirmation of the 18 months prognosis, again ask the question.

You should address any SAR , FAO Data Controller, at the address of the Benefits Office dealing with your ESA.
Put SAR DPA1998 in front top left corner.

Unfortunately the DWP ignored my SAR request for all my documentation and in their wisdom only sent the report from my doctors... So after waiting 41 days for the information I was not happy
