filling in the DLA to PIP FORM was created by maggieomx
iv received the DLA TO PIP form , but whats worrying me is I haven't any doctors reports or anything of the sort to send with my big form, so just the form will be sent on its own ,,,shall I say goodbye to all my CHANCES OF pip now ???

Hi momx,

Many people who have chronic long standing conditions find themselves in a similar situation, especially if they have not seen a Consultant for some time as it has been deemed that no more can be done.

You can provide historical, older evidence, but you would have to make it clear that your conditions have not improved, nor have your resulting limitations/disabilities diminished since the time of the evidence.

You can obtain copies of your your G.P. records by making a Subject Access Request at your G.P. reception.

These records will go back to when you were a child, and should include any test results and letters from Consultants that you have previously seen.

You do not have to contact your G.P. to obtain these, but there will be a charge.

Once you have them, you can copy any relevant extracts and send them with The PIP 2 Form.

It can take up to 40 days to obtain these, so if you haven't got them by the time that you have to return your PIP 2 Form, you should write a comment in the form that you have requested them, and you will forward any evidence gleaned from them to ATOS, as soon as you have them :

Sending Documents to the DWP or ATOS

N.B. A tip, keep the address that you have to return your PIP 2 Form to, and post any further evidence to the same address with a covering letter.

If you could obtain a current letter of support from your G.P. to send with The PIP 2 Form, that should also help.

The Adviser that is coming tomorrow to help with the PIP 2 Form might be able to help with this.

If you can't obtain any supporting written documentation, it is all the more important that details are given in The PIP 2 Form, plus copies of any repeat prescriptions, etc.

Remember, you don't receive a PIP Award because you suffer from a particular medical condition, (Except under a few exceptions) it is how the resulting limitations/disabilities affect you on a day to day basis.

thanks bro but im seeing her tomorrow and my forms have to be in before 10th march so not going to be enough time, on the form it asks for my gp name and address and hospital dr whos treating me name ect will pip write to them and get the info ?? or do I need to do it ??

Hi momx,

Even if you supply the contact details of your G.P. or any other treating HCP, with a request that ATOS contact them, they are not obliged to, and seldom do so.

This is why we advise that members should obtain their own evidence/letters of support and send copies of them with The PIP 2 Form.

well im not going to ave time now as im there tomorrow filling form in and its to be in for 10th march?? told you I have a bad feeling about this everythings against me x

Hi momx,

The 40 days allowed for your G.P. practice to provide your medical records is an absolute maximum.

You may obtain them much quicker than this.

It is dependent on when the IT person is available in your G.P. practice to provide copy.

This is generally done by putting the records on a password protected CD, which you can them view on your P.C. and print off any extracts that you need.

As I advised, there will generally be at least a few weeks or so between ATOS receiving your PIP 2 Form back, and actually looking at it and acting upon it.

It could be considerably longer than that, and you can provide fresh evidence right up until the time of the assessment.

You could therefore send copy of any extra evidence on to them after returning The PIP 2 Form.
