Hi eh,
I will merge this ESA query with your previous ESA topics for continuity, please keep all future ESA related queries to this topic.
If the situation is such that your ESA50 is circa 12 months old and you feel that it does not truly reflect you current limitations, see this FAQ :
Invited to an ESA Face to Face Assessment (ESA “medical”) but no ESA50 sent
If you are happy to attend the face to face, travelling time to or from the Assessment Centre should not exceed 90 minutes by Public Transport.
Maximus do provide Taxis door to door but you would have to gain their agreement to this before hand.
You can read what questions to expect in either of our : ESA Claims Guides 2nd & 3rd links down the page.
If you feel that your limitations dictate that you cannot make the return trip to a Centre and you need a Home Assessment you would have to contact Maximus, and they would probably insist that your G.P. FAXES them a letter supporting this.
You should be aware that how you manage the return trip to/from the Assessment Centre will be taken into account as part of the actual assessment.
You may be observed on approaching, leaving and whilst inside the centre.
Assumptions may be made by the assessor with regards to the above.
These assumptions could result in the assessor writing comments/recommendations within the ESA85 medical report which they compile and send to The DWP.
Any decision made as a result of your assessment will be backdated to the start of week 14 of your claim if this was a fresh ESA claim.
Good luck and please keep us updated.

I have had 2 appointments cancelled even though I had sent a doctors letter saying I needed a taxi due to my mobility and balance issues,
I spoke to ESA and Maximus today and neither know what is going on regarding my claim, they both are blaming each other.
Can I put in a new ESA50 form in for them to give them new information.

If the way that your conditions effect you has changed then you can update Maximus about this. You can either complete a new ESA50 or write a letter detailing the changes, although I would stick to a similar format as the ESA50.
You can download a new ESA50. See
Using The PDF Version Of The ESA50 Questionnaire
