Can I take notes at a PIP assessment? was created by sue orr
on 5.1.18 a HP is coming to make a PIP assessment of my daughter. I completed the form as her mother and appointee because due to mental incapabilities she is not able to do so herself. Can I take notes of the meeting which will be attended by her partner and support worker and have them countersigned in the presence of the HP?

My full name is showing, how can I stop it?

You can take notes without restriction but I'm afraid I don't know what you mean by having them counter signed? Neither the assessor or any other employee of the Assessment Provider is going to certify your notes.
