Your ESA guides (physical and mental) state help with ESA50 or UC50 questionnaires. The ESA form application I downloaded is a ESA1 02/17. Do the guides apply to this version too?

Are you applying for ESA? If you are then you need to complete an ESA1 form first. This assumes that you are not in a Universal Credit area. I'm afraid our guides do not cover the completion of the ESA1 but the questions should be all self-evident, if you have any issues then reply to this post and I will do my best to help.
If you are not applying for ESA, you have already done so, have an existing award of ESA, then you should not be completing an ESA1.
