Hello, any one else had this form? Apparently it’s to determine whether I’m entitled to extra payments called premiums? I’m very anxious I’ll put my ESA in jeopardy iff I fill out the form incorrectly- I have no savings and don’t have a partner with savings which is what the form seems to be about.......I’d really appreciate any feedback/ help. Thank you
