If you have already posted off your esa50 form who do you post further medical reports to, your guide was'nt clear. The C.A.B. guide says send it to the centre for health and disabilty assessments.
Is that correct? or do you post to the DWP Wolverhampton black hole.

It needs to go to CHDA and if you have received an appointment for an assessment the venue address.
