I suffer from quite a few illnesses for which I have to go to quite a few different NHS departments, I don't know if I need to ask all the various specialist departments for my medical history or if my Doctor will be able to provide it. I have been in the support group for the last ten years but I received a form to fill in for ESA, nothing has changed in my conditions apart from getting progressively worse so I was wondering if it would be acceptable to write ' no change ' in the question boxes that had been previously completed in 2009
I would start by speaking to your GP about the support they can provide you. Your consultants should have written to your GP after each appointment, these letters will be in your medical records at your Surgery.
You can't say "no change" on the ESA50 as it's very likely that those assessing you will not have any access to any of your previous forms and evidence, so they won't know what you said previously.
You need to complete the ESA50 in detail and provide evidence as you can, even if it has been sent before.
