Yes, you can request an MR by phone.

Did you expect to receive an Enhanced award for Daily Living and/or a Mobility award?

Sorry the answer to your question was yes.i was expecting some help with mobility side of things.as per previous ftf 2014 got 4 points for prompting to leave house.case manager confirmed hp used info from that assessment which was so long ago it cannot be used.i sent 8 additional pieces of info, and letter from doctor which I sent secure post, but apparently they seem to have lost this evidence.the ftf seemed ok,but when got copy of report were lots of questions I wasn't asked, contradiction and biggest and most traumatic for me was I was supposed to be leaving ftf going to mum's for dinner.my mum is dead.why would hp feel that lie would be relevant.just got ESA form now.no doubt be seeking more advice.hope this makes sense as I am really confused.thanks

You can always send in a written request for an MR, especially if there are things that you want to explain or add to what has was said on the phone call.

If you do tackle the report then try and stick to issues that lead to your scoring points but remember an increased award will only be made by your showing that you meet the criteria for one.

The Going Out activity looks at three things.

Planning a route - this is primarily a cognitive or sensory (e.g. blindness) activity. You are being asked about the problems you would have with working out how to get from one place to another, you do not need to be able to follow the route that you are planning.

Undertaking a Journey - this is to do with mental health issues such as agoraphobia and social anxiety and is concerned with you leaving the house to go somewhere, they will be interested in the things that stop you doing this. You need to show that you would suffer "overwhelming psychological distress" to meet the criteria.

Following a route - This activity about the problems you would have navigating a route. So are there problems; cognitive, sensory or mental health issues that would prevent you from doing this? This is different from undertaking a journey, in fact, if you cannot undertake a journey then you will not score points for following one and vice versa.
