Supporting evidence was created by max151
I am filling in form for my mum. Her 3 year review. I don’t have much supporting evidence. From over the past 3 years since her last one. Is it worth me sending evidence and reports from 3 years ago? Or will there be no point? My mum suffered a very severe brain injury and her condition is about managing it rather than treaments. So we don’t have much contact with the hospital now.

Hi Max151

Yes it is worth sending in reports from three years ago. Make sure that even if you answer a question and say her condition is unchanged, make sure that you give the details of how her condition affects her against the criteria. I would also consider adding a letter from you and/or another relative who has intimate knowledge of her daily living and how her brain injury affects her.
