Many congratulations on the award.
Based on what you have written it sounds the DWP have revised their previous Decision in your favour, so you should contact them for a Decision Letter, although I would not be surprised if they claim that the letter detailing your payment changes was intended to perform this function. You can insist that a formal Decision Notice is issued.
In regard the missing document, it is up to you, but these documents are regularly destroyed every 14 months and it quite unlikely that they will be referred to in any future assessment.
The Disability Income Guarantee is actually referring to the Enhanced Disability Premium which is paid to members of the Support Group who are receiving ESA(IR), it is also paid if a claimant is receiving DLA Higher Rate Care which is probably why you have seen references to DLA.

Will their Decision Notice include how many points, and for which descriptors, I was awarded as well as the award length/prognosis?

It is unlikely, however, there should be a new LT54 which may indicated on what basis the Decision was made, which you can request from the DWP.

Regarding the LT54 :
See this extract : "LT54
The LT54 (DMACR) is completed by the Decision Maker with respect to : Claims, revisions, supersessions, etc, determination of questions and reconsiderations.
Part 3 of this form is used for an IB/IS/SDA transfer. It contains the claimants Conversion to ESA decision, (LCW placed in WRAG) or (LCWRA placed in Support Group). It gives a written explanation of the "Conversion Decision", and details which relevant legislation and regulations have been applied under "The Law".It states the effective date of the decision. It may also include the length of the award, before it should be reviewed."
