hi I have been taken off esa I am appealing in the meantime I was told I can reapply as it has been longer than six months and I am still waiting for a hearing date as I am not fit for work, so I am just started to fill the form in and it has not asked many questions about my disability is this the right form the right form ESA1

The ESA1 is used to apply for ESA, it is used to check whether you are eligible for the benefit, if you are then an ESA50 will be issued which is where you describe the problems you have.
