I would expect you to receive a PIP2 form within two weeks of your making the call, if you have not received it by Monday I would give the DWP a call to find out what is going on.

However, mine took 5 weeks to arrive and it was dated before they could possibly have sent it too.......

The timescale for my PIP application has been as follows -
Called for application form 14th March, received and returned by 21st March. Received by them 26th March.
Received appointment letter for NW11 for 15th April.
Cancelled this and new appointment made for Luton – 8mls away, for 14th May.
