Anyone know if it is ok to submit, for Question 5 - Grounds for Appeal, a typed, separate piece of paper rather than have to write it all out by hand?

I would put a one line summary of each point on the form, for example; I am unable to walk 20m reliably and then more detailed information on one or more attached sheets, make sure that you include a reference to the attachment on the form and staple or attach everything together so it can't get lost.
