Applying 4 PIP- filling in appointee form advice was created by MummyBear
PIP sent a form asking whether my autistic son can act on financial maters himself – (the answer is No) - and if not, whether I would accept to be his appointee - (Yes) - I filled it in, signed and sent to the address provided - have proof of posting. Now I received the same form 3 more times - asking me to fill it in or they would disallow the claim - I called PIP - they said they received a blank form ????
What can I do so my next form wouldn’t turn out blank?
So I am thinking, did I write something I oughtn’t?
Is there a special guidance on how to fill this form?
Who should sign it? Should we both sign it?
How to approach the section “What else would you like to add to support your claim for PIP”?
I explained that he lacks attention and organisation; he wouldn’t understand and wouldn’t even attend to these financial matters.
In examples, I explained how runs up a bills borrowing from people and from the cafeteria at school
Please give me some clues...
