I think I might have asked a related question before. I've been getting Assessment rate ESA since October 2013 (backdated for 3 months). In November 2013 when I was in hospital my parents got a letter saying that I should return the ESA form that they had sent me. As they had not sent a form my Dad phoned and the DWP said they'd send one/another one. I was never provided with another ESA form so just let things be.

I'm now wondering (14 months later) whether I should just fill in the form that's available online or whether my Dad should phone to ask for another?

Yes, you did pose the same query around 4 weeks ago, and Gordon did offer advice on your options : Here.

It is entirely your decision to make, you can either contact ATOS to find out the status of your ESA Assessment, or you can download, save, fill in and send in The PDf Version ESA50 to ATOS.

You should be aware that Maximus are taking over The ESA Assessment process from ATOS in March this year.
