Please please could someone help me my Daughter has Prader- Willi Syndrome which is a genetic condition have had the dreaded renewal forms since Dec yes Dec 2014 just don't know how to complete , She fits into the mental and physical category where do i start she lives in a specialist residential home but comes home to stay frequently . Last renewal had appeal then tribunal then succeeded on supper-cession !!!!.
Any advice where to start DLA ends 22nd April.

The first thing to say is that you must return these DLA forms to the DWP before 22 April or your daughter's claim will be closed with no right of appeal. Send the forms using a tracked post and call the DLA Unit after they have arrived to confirm receipt.
Our DLA Claim guides are on the following link, you need to complete the form in detail as if it is a new claim as you cannot rely on the those reviewing it to look at the previous information.
