ESA50 FORM

I am currently completing the new ESA50 form in anticipation of a reassessment. The boxes are much smaller and I cant reduce the size of the font so have had to added more attachments.
I notice that the form runs from page 2 to page 20. However page 20 concerns 'Cancer Treatment' and fortunately I do not have this condition. As I have to give page numbers to my attachments do I have to include page 20 in the completed form or can stop at page 19 and number my attachments from that page?

Hi dr,

I would include all pages of The ESA50 as they are part of the questionnaire as a whole.

You could simply write N/A (Not Applicable) if that is the case.

You could still include it in your index and write the same.
