my claim form for pip was filled in incorrectly

my claim form for pip was filled in incorrectly was created by creamcake
what can I do shall I cancel the claim and reapply or ring them and point out what has happened or can i point this out at the medical.
Just found my claim form was incorrectly filled in by the citizens advice office they did the paperwork for me on the 17th April 2015 [ I had a nosebleed while i was there so I was a little distracted and i think he must have wanted me out as soon as possible ] I was given the form to sign and he posted it for me I was not given it to read through first, when I got home I realised he had not given me the first page back with my claim number on. Saturday morning I got a letter from Atos for a medical on the 6th may very quick great I thought. rang citizens advice today to see if they had copied the claim so I could have a read through it. to my surprise found he had ticked no to almost every question despite me taking lists with me about how things affect me plus answering his questions I have polyarthritis diagnosed in 1991 spinal stenosis diagnosed 2010, carpel tunnel diagnosed 2011, diabetic diagnosed 2012, cellulitis 2014 incontinence 7 years, dizziness and high blood pressure and extremely swollen feet and ankles with my heels and big toes getting deep cracks for over 7 years making walking a real problem. the arthritis is now also in my hands I am constantly dropping things, he has not filled in the other information bits e.g. to say I can't wash my back or feet myself. another example is question 13a i said yes he put no but on q14 extra information has put cannot follow route of unfamiliar journey needs niece or daughter with her how contradictory is that. no wonder he did not let me read it or give it to me to post, I feel so let down by them how could they put totally different answers to what i gave them and not even fill in the other relevant information that he asked me about. when he first started he said I could not get pip because I got income support.
any advice would be appreciated thank you.

All posts have to be approved by Moderators before they appear in the forum, we are normally online between 2:00pm and 3:30pm and 8:00pm-9:30pm.
You don't say whether this is a completely new claim for PIP or whether you were invited to claim PIP while receiving DLA. If the latter then you cannot withdraw your claim without your DLA claim also being closed.
You can continue to supply information up to the day that you are assessed so there is an opportunity to correct the error on your form by supplying your answers to the questions in a separate letter. I would stick to the same format as the form so it is easy for those reading it to cross-reference your answers and you should obviously include a covering letter explaining what has happened.
If you do this then send a copy to the DWP office that is dealing with your claim, if you have been contacted by an Assessment Provider by the time the letter is ready then send a copy to them and finally, take a copy to any assessment that you have and ask the assessor to read it before proceeding with the assessment and to include it in your file.

If this was a new claim from scratch then you can withdraw it at any time with no fault and make a ne claim for PIP, obviously a new claim will only have effect from the day that it is made so you will lose any time that you have already invested.

Hi. this is a new claim and I have my assessment on wed the 6th may so I will do as you advised and send a revision of the incorrect ticked questions plus include notes on the effect on me. also I have just had a call yesterday to say I have to see spinal specialist on Tuesday so I will ask them if they can give me a report for assessment on Wednesday. thank you for your help very much appreciated I was really in a panic on Monday night.
