Hi bd,
You do not have to provide details of why you have changed address, you simply have to inform them of your new address, and the applicable date of your change of address.
If you start sending medical letters in, they may take this as you reporting a Change of Circumstances regarding your medical conditions which could instigate a review.
Keep the letter/s basic and to the point !
For DLA send it to the address on your most recent DLA letter and for ESA do the same, the address on your most recent ESA letter :
Sending Documents to the DWP. Reporting a Change of Address only should not instigate a review.
