esa50 medical form

Hope you had a nice rest for 3 days.
I have now been in the SG esa for nearly 4 years, (unbelievable) so am sure I will be called for a medical soon. I am completing the form a bit at a time on a daily basis My question is (I think I know the reply you will give) on the physical questions i.e. 'putting something in top pocket,' it would depend on what the something was, I have found this on all the questions, I cannot answer yes or no, on a lot of the physical Q so all my answers are going to b 'it varies', Is this ok, and yes I will go into why it varies on a separate sheet, Is this the right thing to do.

Yes thanks, the break was well needed !

As we always advise, if none of the tick box options fit in with the limitations that you actually suffer in carrying out the activity, you do not have to tick them.
Simply write something like none of the tick box options are applicable please see my comments below in the text box. (And/or on separate A4 page number xx of xx)
i.e. Don't be constrained by the available tick box options.

One more question, I sent in copies of medical consultant reports on my Last ESA 50 in 2011, should I re-send, these again, or just state "as in my last esa 50 of 2011". I do have up to date reports, I would rather not have to re-send the 2011 reports, would this be appropriate.

Hi r,
As with all reassessments, you should treat this as if it was your first assessment.
Do not rely on the fact that the assessor will have access to your medical history, they may not even be aware which group you are in so you should inform them.
Obviously current evidence would carry more weight, but if you are to use "old" evidence make sure that you state that your medical conditions have not improved since the time of your last ESA Award, nor have your resulting limitations diminished to such an extent that you no longer qualify for an SG Award.
Reiterate how and why you still qualify for an SG Award !
You could also state that you have not learned to adapt to, or cope better with the resulting limitations, all the above being the case of course.

Thank you Bro 58, for your reply, will follow your advice . However, your answer has had me thinking. If the person carrying out the wca, does not have any knowledge of my previous medical history, how, do they decide whether or not I would need to be seen by a doctor, as I was on my last ftf wca,
I bet Its a case me turning up (when they ask) only to be told I would have to make another appointment to be seen by a doctor. My Health condition's (as stated in the DWP handbook) back in 2011 stated this. Sorry for being a pain.

Information on the current state of your medical condition/s and resulting limitations with respect to whether a face to face should take place and what type of HCP should carry out the assessment should be gleaned from the ESA50 (or comparable Pdf ESA50) that you receive, fill in and return prior to your reassessment.
